Error 404 when submitting form

Hi,
I have been using this excellent plug-in for around three years and have been delighted with it.
However, when trying to submit the completed form I receive a 404 error and the booking hasn't been made.
Any suggestions please?

Hi Mike, sorry for waiting reply. That can be due to caching? Did you change something lately?
