have you tried to turn on Compatibility mode? It’s on Settings > Customize page.
Compatibility mode is not worked for me either. I have tried IE and Chrome.
I just saw that you only forgot to do the refresh and after that it was working fine 🙂
I have the same issue. Compatibility mode does not resolve situation. Neither re-installing of add-on did. Please, help. Greg

You mean like in Setting -> Tools -> Error log? That is empty.
I also noticed that e-mails are not sent (both to admin and customer) even though appointment was added to dashboard. And after clicking “Submit” button, there is spinning wheel over it with no further progress. Test e-mails are sent fine. I suspect faulty custom form field makes script error?
So I removed it again. If used with not changed “Custom form fields” it works. If I try to change anything – remove one of them, add new and modify default values – I get errors.
