Please help this is my second request. It will probably be simple for someone with expertise. When a consumer submits the form for an appointment they get the following email confirming the appointment. The description or Model are not being sent to the person in an email it shows as follow #description#instead of the input. also below this email I have the code I am using.

Hi, can you please check the lists of available tags there? It’s above text editor field. Can you paste it here?

At least in my case, all custom form fields are included in lowercase letters, so maybe that’s why #Description# and #Model# won’t work. Try #description# and #model# in your mail template.

Yes, that should be it. Replacing tags is case sensitive.
