I have loaded the bootstrap string into this page, however nothing appears when published. https://californiahomeequityforseniors.org/book-an-appointment/ if anyone has any answers, I would appreciate it. Thanks

Hi Kevin, sorry for waiting. Looks like styles issue because all elements are there but without proper styles. Have you tried bootstrap version of short code? Best regards, Nikola
