How do I include a custom form field in the Mail Notifications? Adding a “select” field does not give a place for a slug to reference.

Hi, slug for that type of field will be fixed asap. In next version you will be able to see slog for select field.

I see there was an update, which I installed, but now the form to collect the info (name, email, etc) isn’t showing up. I select the employee, the date, the time, and the calendar just fades a bit and the form doesn’t load. It does this in FIrefox and Chrome.
