I posted a problem a couple of weeks ago and you said it would be fixed in the next update: https://easy-appointments.net/support/including-custom-form-fields-in-email/ I see there was an update, which I installed, but now the form to collect the info (name, email, etc) isn’t showing up. I select the employee, the date, the time, and the calendar just fades a bit and the form doesn’t load. It does this in FIrefox and Chrome. Screenshot: https://prnt.sc/12fsskw
