Support forumCategory: BugsError 404 when submitting form

I just purchased the connect extension and set everything up. However, when submitting the booking form, I get a 404 error, and the appointment doesn’t get booked in. But it temporarily reserves the time slot.
The 404 page URL looks like this: (changed my email for privacy)
I’m also not getting any emails, and the email test in settings doesn’t work but also doesn’t show any logs.
When I use the legacy form, it works and I get a success message on the page without any redirect. Appointment is booked and syncs to Google calendar, however also no emails.
But I need to use the new bootstrap version because of the options.

I’ve disabled all caching and SEO plugins etc.

Hi, I was able to book appointment. Can you please check did you get any email?

Hi Nikola,
Thanks for your reply. I didn’t get your booking, no email and it’s not in the EA Appointments list. Did you try the new (bootstrap) or old form?
New form is at: 

I got the emails working now, using SMTP instead of PHP method. On the old form I can book and getting the emails. On the new form still the 404 error when submitting.
