Customer needs to fill Location

Hello,
We are using this plugin so clients can request an in home consultation. I want the address to show up on the email that is sent to us when the client requests the consultation. How do I make that happen? I am using #location_address#, but that shows the location of our home office?
I want the customer to be able to provide an address and for it to translate into the notification email.

Hi you need to add custom form field
Client Location and then inside template put something like #client_location#.
