2 column shortcode not working!

Please see my appointment calendar at https://saddlebackeye.com/schedule-a-free-exam/
I am using this shortcode: [[ea_bootstrap width="800px" scroll_off="true" layout_cols="2"]]
But the calendar still loads into a single column. How can I override any conflicting theme code to make this 2 columns. NOTE: It WAS 2 columns days ago and i do not believe I've made any updates that would have broken it.
Help?!

Hi, it should look like this: https://easy-appointments.net/responsive-two-columns/ but have you added some style or change theme on your site. Sometimes it can have conflict with styles of EA. Best regards, Nikola

No, I didn’t add anything. Is there any way you can look at the css and provide guidance on how I might fix it? Thanks, Nikola!!
