Turn on checkbox Label

Why is there 2 checkboxes? When I check "Turn on checkbox" and then enter the text for the Label, I get 2 separate checkboxes. But if I uncheck "Turn on checkbox" I don't get the Label. I only want the User to have to check 1 checkbox.
