Support forumCategory: QuestionsCustom emails not working

Hello, I have filled in the custom email notification and it is only displaying the default notification. I entered:
Pending Appointment Request:
#worker_name#
#date#, #start#
#service_name#
#email#, #name#, #phone#, #message#
 
 
However, it is only displaying the default table. Thanks in advance for your help!
